Please fill out the form below for each person on your team then click "Save & add another person." There are up to 4 people per team. If you don't have all 4 people, select an individual registration for each person.

You also have an option to sponsor a team for $500, $1000, $2000 or $5000!!! If you are sponsoring a team, please select the sponsorship level you'd like and enter each player's information by clicking "Save & add another person." Each subsequent player's "ticket" option will be free.
